HOUSING STABILIZATION SERVICES (HSS)

About HSS

Housing Stabilization Services (HSS) are for people with disabilities, including mental illness and substance abuse disorder, who are:  

  • Homeless or at risk of becoming homeless 

  • Living in institutions or other segregated the community, and settings, or are at risk of living in those settings

 

The purpose of these services is to:

  1. Support an individual's transition into housing,

  2. Increase long-term stability in housing in the community, and

  3. Avoid future periods of homelessness or institutionalization.

TRANSITION SERVICES: Transition services helps a person plan for, find, and move into housing and may include:

  • Help thinking about housing options

  • Help resolving barriers to accessing housing

  • Help with applying for benefits to afford housing

  • Help with the housing search and applications

  • Developing a budget

  • Help finding resources to move

  • Understanding a lease

  • Making the new housing safe and ready to move-in

 

SUSTAINING SERVICES: Sustaining services helps a person keep their housing after they have moved in and may include:

  • Education on rental rights and responsibilities

  • Help improving relationships with property managers and neighbors

  • Training on how to be a good tenant; lease compliance

  • Problem-solving to maintain stable housing

  • Help identifying supports and resources in the community

 

CONSULTATION SERVICES: Consultation Services helps a person who does not have Medical Assistance (MA) case management develop a person-centered plan that addresses their needs, wants, and goals for living in the community and may include

  • Developing a housing focused person-centered plan based on the person's needs, wants, and goals for housing

  • Helping a person make an informed choice in their housing transition or sustaining services provider

  • Offering resources related to non-housing goals

  • Coordinating with other service providers already working with the person
